Lower School Trip Permission Form

After completing and submitting this Permission Form you will not be asked to complete this form again whilst your child(ren) are at the Lower School.

For one-day field trips the teacher will notify parents at least two weeks in advance.

For residential trips parents will be provided the logistics at least two weeks in advance. The dates of the residential trip will be shared at the beginning of the year or when the dates are confirmed with the external provider.

The communication will include the destination of the field trip and essential logistics.

I agree to contact the school immediately if any student contact or health information changes by completing the Information Update Form.

Permissionrequired

I hereby give permission for the student named above to participate in the trip indicated.

In the event of his/her subsequent withdrawal from the trip I am obligated to pay for any non-recoverable expenses incurred by the School.

I understand that it is my responsibility to provide for him/her appropriate accident and health insurance.

I give permission to the chaperone(s) to authorize, on my behalf, any emergency medical care, including surgery, which on the recommendation of qualified medical personnel may be deemed necessary. In such circumstances, the chaperone(s) will seek to advise me (or the emergency contact designated above) first, and if I am unreachable, then at the earliest possible convenience.

I understand that conduct by student participants in this trip shall be in accordance with standards specified in the Lower School Handbook. I understand that violations of these standards of behavior, as determined by the chaperone(s), may result in the student being sent home early at my expense. In such circumstances, I understand that it is my responsibility to ensure that I, or the nominated guardian, is available.

ZIS, its teachers, administrators, employees and associates wave its liability as far as in line with Swiss law in respect of all personal injuries or property losses which the student may suffer arising out of, or connected with, his or her preparation for or participation in the aforesaid program or activity.
